Liability Waiver

In consideration for the Participant being allowed to participate in the above described hockey activity, related events and activities (collectively, the “Activity”), the undersigned parent and/or legal guardian acknowledges and agrees as follows:

The undersigned parent and/or legal guardian acknowledges that the Participant voluntarily elects to participate in the Activity knowingly and freely given the nature of the Activity and acknowledges that participation in the Activity involves inherent risks including but not limited to bodily injury, property damage and death. The undersigned parent and/or legal guardian and the Participant understand that the Activity may involve the use of equipment and machinery, interactions with other participants and customary Activity activities. The undersigned parent and/or legal guardian and the Participant assume and accept any and all risks, whether known or unknown, minor or serious, that may arise out of the Participant's participation.

The undersigned parent and/or legal guardian declares that the Participant is physically sound and not suffering from any condition, impairment, disease, infirmity or other illness that would prevent participation in the Activity. The undersigned parent and/or legal guardian acknowledges that it has been recommended that the Participant consult with a physician prior to participating in the Activity. The undersigned parent and/or legal guardian acknowledges that the Participant has either had a physical examination and received a physician's permission to participate or that the undersigned parent and/or legal guardian has decided to allow the Participant to participate without the approval of a physician and assumes all responsibility and risks for the Participant's involvement in the Activity.

The undersigned parent and/or legal guardian and the Participant understand that Hockeyology, its directors, officers, employees, agents and representatives, other participants and assigns will not be liable for damage, theft, loss or other issues with personal belongings, valuables or electronic devices brought to the Activity. The undersigned parent and/or legal guardian and the Participant understand that if the Participant brings any personal belongings, electronic devices, money or other items of value, the Participant does so at his/her own risk.

The undersigned parent and/or legal guardian and the Participant agree that the Participant will abide by and observe all policies, rules, regulations and terms of play for participation in the Activity, demonstrate good sportsmanlike conduct at all times and agree to abide by any posted rules and regulations in any facility or location of the Activity.

In consideration for the Participant being allowed to Participate in the Activity, on behalf of myself, the Participant, his/her personal representatives, heirs, next of kin, successors and assigns, the undersigned parent and/or legal guardian agrees as follows:

TO INDEMNIFY, DEFEND AND SAVE HARMLESS Hockeyology, its directors, officers, employees, agents and representatives from and against any and all losses, claims, actions, damages, liability, costs, or expenses of whatsoever kind or nature, arising from any accident, injury, accidental death or damage to property that is caused or results from the Participant's participation in the Activity.

TO RELEASE, REMISE, WAIVE AND FOREVER DISCHARGE Hockeyology, its directors, officers, employees, agents and representatives from any and all past, present and future losses, claims, actions, damages, liability, costs, or expenses of whatsoever kind or nature, arising from any accident, injury, accidental death or damage to property that is caused or results from the Participant's participation in the Activity.

I have read this waiver and release of liability, as well as the ZerOne Code of Conduct, and I agree to this waiver, Code of Conduct, and release of liability freely and voluntarily without any inducement, assurance or warranty being made. I acknowledge that this agreement is binding on me, my heirs, executors, administrators, and representatives.
